Stay Well Home Health provides in-home care to those who may need a little extra help once they leave our skilled nursing/rehab care. They provide physical, occupational and speech therapy, companion care, homemaking, personal care and assistance with activities of daily living.

Hospice Care Partner
The main focus of hospice care is to relieve pain for those in the last six months of life. Hospice care focuses on comfort, rather than a cure. They provide care in all of our skilled nursing locations allowing your loved one to stay in a familiar and comfortable space.

Palliative Care Services Partner
The CareBridge Palliative Care team helps to reduce the pain, stress and discomfort of chronic and serious illnesses. The difference between hospice and palliative care, is while the main focus of each is to ease the symptoms of incurable illnesses such as cancer, heart disease, lung disease, dementia, kidney failure and more — hospice care is typically reserved for those with less than six months to live.
