For those facing a life-limiting illness or injury, or an end of life frailty; we provide a team-oriented approach of expert palliative and hospice medical care which includes pain management, and emotional and spiritual support expressly personalized to each patient and/or resident’s needs and wishes.
While we work side by side with many Hospice organizations and will continue to do so, our many years of experience has supported that our residents benefit from the care and support provided from the affiliation of our nursing facilities with Hospice of Southwest. - Palliative care can begin at diagnosis and at the same time as treatment to help with medication management and care.
- Hospice care begins after treatment of the disease is stopped and when it is clear that you are facing the end of life to assist with the physical, emotional and psychosocial aspects of dying.
- Hospice staff can provide daily supportive care in our nursing facility along with continuous care and support of you and your family at the bedside when it is most needed.
