Oak Creek Terrace Nursing, Rehab & Assisted Living is centrally located in the heart of Dayton, Ohio, near Bigger Road and Rahn Road. We provide an array of rehabilitative and nursing services as well as Assisted Living one-bedroom apartments to all of Greater Dayton senior population. Our convenient location in Kettering, Ohio is nearby most major hospitals, making it an easy transition when being discharged and rehab is necessary. We can admit new patients not only upon discharge but also from the emergency room, doctor’s office or home. Contact us for more details.

I have had my mother in other nursing homes and this one is definitely a six star nursing room. She is really happy and they take excellent care of her.
My 95 year old mother suffered a fall and needed care. Oak Creek Terrace had the best ratings, so we wanted Mom there in the memory wing. It’s a very positive atmosphere. There was warmth, a family feeling. Tracy is the leader, but everyone echoed the friendliness. The rehad staff has been fantastic.
Oak Creek Terrace took excellent care of my Dad. Their rooms are large, their staff was kind and courteous. I could always call them and get my questions answered. The food was good. My Dad could walk around the facility, to the dining room and to visit with friends without any problem towards the end of his stay. So the Physical Therapists did an excellent job getting him back on his feet.
Oak Creek Terrace is and excellent facility. A very positive experience for all of us. I definitely recommend Oak Creek Terrace.
